When you begin as a new patient with Dr. Scott or her Nurse Practitioner Elise Lewis, you will need two appointments to start your treatment plan. In the first appointment, which is approximately an hour long, you will discuss symptoms you are or have been experiencing, what issues you would like to address, your health history and more. Any current labs or tests from another doctor/clinic must be sent ahead of your appointment. You can fax them to 972-695-4001 or scan and email them to email@example.com.
Based on this initial assessment, Dr. Scott or N.P. Lewis will determine what lab panel is appropriate based on your concerns, symptoms and history. You will be given a lab requisition with lab instructions to have your labs drawn as soon as possible. This must be done at least 2 weeks prior to your second appointment as it takes some time to get the lab results in. Please check with your insurance provider to verify who their preferred lab company is (usually Quest Diagnostics or LabCorp). You should also find out if they will cover the cost of your lab tests. If you do not have insurance or your insurance will not cover this lab work, we have a special discount program (through Quest) for patients without insurance. You may have your blood drawn at the lab of your choice.
In your second appointment the provider will explain to you your test results and how these relate to your symptoms. She will lay out a treatment plan and prescribe anything that is indicated. Your second appointment will be a prescribing visit. If you would also like a thorough supplement review, please bring all supplement bottles you take to your second appointment.
As well, please understand that getting your hormones balanced and other wellness steps we will take are not a one-time thing. Follow-up appointments will be needed to monitor and balance your levels and to possibly address new issues. Shifts in diet, exercise, stress levels and other lifestyle changes and factors can affect your hormones and your body. After your first two initial appointments, Dr. Scott likes to see you every 3 months for your first year. After this, one can usually shift to every 6 months for continued maintenance.
PROGRAM AND FEES
Your first two appointments will entail approximately 2 1/2 hours of quality time with Dr. Scott, for a combined fee of $795, with the option of presenting your insurance for your lab fees. When you book your appointments you will be asked to put down a deposit, which simply goes toward the cost of your two-appointment package. Your $250 deposit will be applied to the $795 fee, leaving a balance of $545 due at your initial appointment. Your second visit is then already paid. Additional visits after this will be subject to our a la carte rates of $350 for an hour and $175 for a 30-minute appointment.
Nurse Practitioner Elise Lewis follows the same protocol as Dr. Scott for a fee of $595 (which covers your first two appointments), with the option of presenting your insurance for your lab fees. When you book your appointments you will be asked to put down a deposit, which goes toward the cost of your two-appointment package. Your $250 deposit will be applied to the $595 fee, leaving a balance of $345 due at your initial appointment. You will pay nothing more at your second appointment as you will have already paid. Additional visits after this will be subject to our a la carte rates of $300 for an hour and $150 for a 30-minute appointment.
Your hormones and supplements can range between $150 and $450 for a 90 day (3 month) supply, depending on quantity and delivery method.
In terms of your office visit with the doctor or nurse practitioner, we are an out of network provider. The reason for this is that in order to get to the root cause of the problem and provide optimal treatment we find we need more time with our patients and more treatment options than insurance companies are willing to work with.
No matter what type of insurance you have, your insurance will frequently be able to cover all or a majority of your lab work. Labs written from Renewed Vitality are typically covered the same as if they were written by your regular doctor which is 90 to 100%. You should however ask your insurance company how they cover the labs and which lab network is preferred. We will make sure that any labs ordered are done through this preferred provider.
If your insurance provider does not offer to cover a majority of the lab work or if it is being applied to your deductible we recommend that you use our discount program that we offer through Quest Diagnostics.
If you have a PPO type of insurance plan, it is possible that your insurance company will reimburse you for much of your office visit costs. This depends on your insurance company, what type of plan you have and your deductibles. You will pay for the appointments at the time of service and we will provide you with the necessary papers (upon request) for you to file for reimbursement with your insurance provider.
If you are on an HMO plan, most likely your insurance will not cover any of the costs of your office visits, but will likely cover all or a majority of your lab work.
To find out what your insurance will cover we encourage you to call the customer service number on the back of your insurance card and ask a few basic questions. First of all tell them that you’re going to see an out of network provider. This means someone that is not on the insurance list of doctors. That said most PPO plans understand that people would prefer to see a doctor that they researched or that comes highly recommended from a friend. Ask your insurance company what the deductible is and what percent they will reimburse for the office visit with an out of network doctor or nurse practitioner.
When talking to your insurance customer service, tell them that your out of network provider is going to order very comprehensive lab work. Ask them specifically what percentage of the lab costs they will pay. We offer a self pay discount program for labs, which is a $500 flat fee for the full set of lab work to be done.
NEW PATIENT FORMS
When you book your first appointments as a new patient we will email you your new patient forms. This will include: “Health Questionnaire, HIPAA Notice of Privacy Practices & Patient Acknowledgement, E-mail/Text Message and Internet Consent, our Informed Consent Form, as well as our Practice Policies and Agreement”. These are the forms you will need to fill out before your appointment. You do not need to fill in any of the questionnaires or health information in the patient portal.
You will want to print, complete and bring these forms with you to your initial appointment. Please let us know if you have any questions regarding your forms.
Our ability to draw effective conclusions about your present state of health and how to improve it depends, to a significant extent, on your ability to respond thoughtfully and accurately to your written health questionnaire and those posed during your consultations by Dr. Leigh Ann Scott. Health issues are usually influenced by many factors. Accurately assessing all the factors and comprehensively managing them is the best way to deal with these health challenges. Your careful consideration of your health questionnaire will help to identify underlying causes of illness and will also assist us to formulate a treatment plan and gain effective use of your scheduled consultation time.
