Our office and staff welcome you to the office of Leigh Ann Scott, M.D. We look forward to working with you and helping you to achieve optimal health.
When you begin as a new patient with Dr. Scott or one of her Nurse Practitioners, Elise Lewis or Sue Miller, you will start your treatment plan with one 2-hour long appointment. Future appointments are one-hour long.
You will be sent a very thorough Health Questionnaire to fill out. You can fill this out online or by hand.
Our ability to draw effective conclusions about your present state of health and how to improve it depends, to a significant extent, on your ability to respond thoughtfully and accurately to your written health questionnaire and those posed during your consultations with your provider. Health issues are usually influenced by many factors. Accurately assessing all the factors and comprehensively managing them is the best way to deal with these health challenges. Your careful consideration of your health questionnaire will help to identify underlying causes of illness and will also assist us to formulate a treatment plan and gain effective use of your scheduled consultation time.
Once you have you booked your appointment and filled in the Health Questionnaire, you will be sent a lab requisition with lab instructions to have your labs drawn as soon as possible. This must be done at least 3 weeks prior to your appointment as it takes some time to get the lab results in. Please check with your insurance provider to verify who their preferred lab company is (usually Quest Diagnostics or LabCorp). You should also confirm that they will in fact cover the cost of your lab tests. If you do not have insurance or your insurance will not cover this lab work, we have a special discount program (through Quest) for patients without insurance. You may have your blood drawn at the lab of your choice.
Any previous labs or tests from another doctor/clinic that you would like reviewed must be sent in to us ahead of your appointment. You can fax them to 972-695-4001 or scan and email them to firstname.lastname@example.org.
In this first 2-hour long appointment, we will discuss the symptoms you are or have been experiencing, what issues you would like to address, your health history and any concerns you have. Dr. Scott or the Nurse Practitioner will explain to you your test results and how these relate to your symptoms. She will lay out a treatment plan and prescribe anything that is indicated.
If you would also like a thorough supplement review, please bring all supplement bottles or a list of these to your appointment.
As well, please understand that getting your hormones balanced and other wellness steps we will take are not a one-time thing. Follow-up appointments will be needed to monitor and balance your levels and to possibly address new issues. Shifts in diet, exercise, stress levels and other lifestyle changes and factors can affect your hormones and your body. After your initial appointment, our providers like to see you every 3 months for your first year. After this, one can usually shift to every 6 months for continued maintenance.
PROGRAM AND FEES
Your first appointment will entail approximately 2 hours of quality time with Dr. Scott, for a fee of $795, with the option of presenting your insurance for your lab fees. When you book your appointments you will be asked to put down a deposit, which simply goes toward the cost of your first appointment. Your $250 deposit will be applied to the $795 fee, leaving a balance of $545 due at your initial appointment. Additional follow-up visits for the first year will be approximately every 3-months and will be subject to our a la carte rates of $350 for an hour. Biannual visits after the first year of treatment will be at a cost of $400.
Nurse Practitioners Elise Lewis and Sue Miller follow the same protocol as Dr. Scott for a fee of $595 (for your first 2-hour appointment), with the option of presenting your insurance for your lab fees. When you book your appointments you will be asked to put down a deposit, which goes toward the cost of your appointment. Your $250 deposit will be applied to the $595 fee, leaving a balance of $345 due at your initial appointment. Additional follow-up visits for the first year will be approximately every 3-months and will be subject to our a la carte rates of $300 for an hour. Biannual visits after the first year of treatment will be at a cost of $400.
Your hormones and supplements can range between $50 and $150 for a 30-day supply, depending on quantity and delivery method.
In terms of your office visit with the doctor or nurse practitioner, we are an out of network provider. The reason for this is that in order to get to the root cause of the problem and provide optimal treatment we find we need more time with our patients and more treatment options than insurance companies are willing to work with.
No matter what type of insurance you have, your insurance will frequently be able to cover all or a majority of your lab work. Labs written from our office are typically covered the same as if they were written by your regular doctor which is 90 to 100%. To find out what your insurance will cover we encourage you to call the customer service number on the back of your insurance card and ask them.
If your insurance provider does not offer to cover a majority of the lab work or if it is being applied to your deductible we recommend that you use our discount program that we offer through Quest Diagnostics.
If you have a PPO type of insurance plan, it is possible that your insurance company will reimburse you for much of your office visit costs. This depends on your insurance company, what type of plan you have and your deductibles. You will pay for the appointments at the time of service and we can provide you with the necessary papers (upon request) for you to file for reimbursement with your insurance provider.
If you are on an HMO plan, most likely your insurance will not cover any of the costs of your office visits, but will likely cover all or a majority of your lab work.
Tell them that you’re going to see an out of network provider. This means someone that is not on the insurance list of doctors. That said most PPO plans understand that people would prefer to see a doctor that they researched or that comes highly recommended from a friend. Ask your insurance company what the deductible is and what percent they will reimburse for the office visit with an out of network doctor or nurse practitioner.
When talking to your insurance customer service, tell them that your out of network provider is going to order very comprehensive lab work. Ask them specifically what percentage of the lab costs they will pay. We offer a self pay discount program for labs, which is a $500 flat fee for the full set of lab work to be done.
